The Scotland midfielder completed the required sporting-health protocols on Wednesday after a controlled recovery from a mild benign arrhythmia detected in early September.
Napoli’s medical staff identified the irregular heartbeat through wearable monitoring equipment used during training and matches. The data prompted an early diagnosis, and McTominay elected to have an ablation procedure intended to remove the issue permanently.
The operation was successful, but the 29-year-old was initially instructed to take two full weeks away from activity. His first work back at Napoli’s training base was limited to gym sessions, individual ball exercises and non-contact drills before he could receive clearance for competitive football.
His availability gives Max Allegri a timely midfield option. McTominay has played only twice this season, both outings coming in Serie A, while Napoli sit ninth with seven points — six behind leaders Roma.
McTominay joined Napoli from Manchester United in the summer of 2024 for about £25.7 million, ending a seven-year spell at Old Trafford. He played a central part in Napoli’s 2024-25 Scudetto triumph and was named Serie A Player of the Year, before helping the club lift the Supercoppa Italiana in 2025-26.
What happens next: Allegri will assess McTominay’s match fitness ahead of Saturday, when the midfielder could be included among the substitutes against Frosinone.
